Treatment of Burnout
Burnout is not laziness or weakness; it is a picture of exhaustion that develops when prolonged stress exceeds the capacity to recover. In the Netherlands it is officially recognised and there is a structured recovery process.
The treatment of burnout progresses in three stages: (1) rest and distance from the source of stress, (2) working in therapy on the patterns that lead to exhaustion (being unable to set boundaries, perfectionism, the need to prove oneself), (3) a gradual and planned return to work. In the Netherlands this runs parallel to the company doctor (bedrijfsarts) process after a sick leave (ziekmelding); ViaNova Praktijk supports this process in Turkish.
The mechanism: how does burnout develop?
The stress system is designed for short loads: load, recover. Burnout is the systematic violation of the recovery window for months to years. The signals come in stages: first sleep is disrupted and the weekend no longer restores; then cognitive symptoms arrive (loss of concentration, difficulty with simple decisions, memory complaints); finally emotional flattening and a picture of 'feeling nothing at all' set in. The critical point: the picture is created not by the workload alone, but by the pattern of coping with that load, being unable to say no, perfectionism, the need to prove one's worth through performance.
An extra layer for the migrant employee
In Turkish speakers working in the Netherlands, burnout often carries an extra layer: the pressure to 'work twice as hard and prove oneself', the burden of not shaming the family, and the belief 'after all this sacrifice I have no right to be tired'. These beliefs delay seeking help and deepen the picture. In therapy this cultural layer is not ignored; it is placed at the centre.
In the Netherlands, burnout is an official process
Unlike in Turkey, a sick leave (ziekmelding) due to burnout is legitimate and common in the Netherlands. The company doctor (bedrijfsarts) becomes involved in the process, and the employer is responsible for a reintegration plan under the Wet verbetering poortwachter. The work in therapy runs parallel to this official process: preparation for the meetings with the bedrijfsarts, the psychological part of the gradual-return plan, and relapse prevention after the return.
The difference from depression
The two pictures can overlap, but the mechanism differs: burnout is context-dependent, partial relief is seen when moving away from work and self-worth remains relatively preserved; in depression the low mood spreads independently of context, and guilt and worthlessness come to the fore. The distinction changes the treatment; in the assessment interview this distinction is made carefully, and if needed the work shifts to a depression-focused approach.
The stages of treatment
The first stage is recovery: sleep, movement and reduction of stimulation, in this stage the pressure of 'self-improvement' is postponed. In the second stage the patterns that lead to exhaustion are worked on: boundary-setting, cycles of perfectionism, the origins of the need to prove oneself. The third stage is a gradual return to work: the increase in hours and responsibilities is done according to plan, and a personal monitoring system is set up for early warning signals. Compressing the three stages is the most common cause of relapse.
Frequently asked questions
How long does it take for a burnout to pass?
Depending on the depth of the picture, recovery is measured in months; 2 to 3 months is realistic for mild cases, 6 to 12 months for entrenched ones. Pressure to return early is the strongest predictor of relapse.
Should I quit my job?
Major decisions made in the middle of a burnout are usually not healthy. First recovery, then working on patterns; a decision to change jobs can only be made healthily with that clarity.
Will a sick leave (ziekmelding) harm my career?
In the Netherlands, a ziekmelding due to burnout is a common and legally protected process; an employer cannot dismiss you for this. The real career risk is the performance and health cost of an untreated, ongoing burnout.
How should I prepare for the meeting with the company doctor (bedrijfsarts)?
Describe your symptoms in terms of concrete loss of functioning (sleep, concentration, error rate) and clearly state your own recovery needs. In therapy you can prepare for these meetings together.
